Potential benefits of anticoagulant doses of low molecular weight heparin in COVID-19: An observational retrospective study

Hypercoagulability is a common complication of the systemic inflammation related to coronavirus disease 2019 creating debate within critical care community on therapeutic utility Low Molecular Weight Heparin (LMWH). We collected data consecutive patients with COVID-19 admitted Emergency Department Castel San Giovanni Hospital, between February 29th and April 7th, 2020. Exclusion criteria were age <18 years, hospital stay <7 days, dialysis who had been transferred other centers for which we could not collect data. Of 257 included in study, 49 (19.1%) died during hospitalization. considered wide set variables as independent (age, sex, comorbidities in-hospital treatments). used multivariate logistic regression model and, being heparin only one therapy affecting survival rate, compared prophylactic LMWH (p-LMWH) Therapeutic (T-LMWH) groups. Kaplan Meier curve showed higher probability T-LMWH difference two groups was statistically significant according log-rank or Mantel- Haenszel test (p< 0.0001). In stratified analysis by ventilation type, subgroup benefited from that non-invasive mechanical ventilation. Using adjusting drugs intake, TLMWH impacting (HR 0.293, p <0.001). No fatal bleeding observed. dose pneumonia 70 associated decrease risk intra-hospital mortality.
